ABSTRACT
Mitra Medika Hospitas is private hospital in Bondowoso City. Mitra Medika Hospital still used manual record when record every single data patients there has in Form Follow Up Outcome Patients, so effect from this problem is losted Form Follow Up Outcome Patient because negligence from workers they borrowed this form out of medical record room and filling room. Consequences didn’t has redudancy data to replace data’s losted. This research method is Qualitative Method with used designing method Waterfall System Model. Steps from this method are Analysis Needs, Design System, Code Program Design, Testing and Implementation, Maintenance Program. Purpose from this research is designing and making System Information Form Follow Up Outcome Patients Mitra Medika Hospital of Bondowoso. Follow Up Outcome Petients System Information will help to decreas problem in this hospital and increas workers performance especially Medical Recorders.
