ABTRACT





Reporting system for diseases that can be prevented by immunization (PD3I) Health Department of Jember is done manually. Data was obtained from 50 units of community Health centers (puskesmas) at Jember area and required to report PD3I data quickly and accurately, which was processed as information for evaluation of prevention and control strategies. PD3I Officer of Health Department of Jember must input the PD3I data from sources into Excel so that it causes multiple workloads and will take a long time to produce the reports. The purpose of this research is to make an application for reporting diseases that can be prevented by website-based immunization of Health Department of Jember as the suggestion for Health Department of Jember to produce better, relevant and accurate information in reporting PD3I activities easily. The system development method was Waterfall method with a qualitative approach, where the process of collecting and retrieving data was done by interviews and observations. Using PHP programming language, XAMPP as a database server, and Notepad

++ for web editors. The results of this study are Reporting Applications that can be Prevented with Website-Based Immunization
