ABSTRACT
The medical record form needs to be designed clearly, neatly and informatively to make it easier for users to understand and filling out forms. Based on the pre-interview survey at Community Health Center of Klabang, the emergency medical record form has items not filled by officers as much as 90%, patient admission 80%, patient out patient status 90%, and 75% has been broken or torn. Also on the form has not shown the exact title of the form, theresno command about how to fill the form. The purpose of this research is to design and create an emergency medical record form at Community Health Center of Klabang. This type of research is a qualitative study by using the instrument to collect data from observation guide, interview guides, brainstorming guides. The variables studied include physical aspects, aspects of anatomy, content aspects, and which is user needs. The result of this research is on physical aspect using HVS 80 gram paper, anatomical aspect mentioning name of Community Health Center at head of form, title of form is "emergency medical record", signature of doctor in charge. The content aspect includes the additional of triage labels, allergy histories, and diagnostic and action columns.
