(Rossalina Adi Wijayanti S.KM., M.Kes) UlvaDwiFaturohmi Medical Record of SudyProgram Department Of Health ABSTRACT Puskesmas Kaliwatesis one of the Puskesmas which used one medical record number for one patient in the registration process. However, the fact is some medical record number duplication still happens. This causing impact such as the patient has to wait longer to get health service, the unsustainability of the patient medical record and the accumulation of medical record document in filing unit. The study used the behavioral theory from WHO which include subvariable such as knowledge, attitude, personal referencess, facilities (visit card, registration book and information system) and the officers habit. The subject of this research is 3 outpatient registration officers and 2 inpatient registration officers. The type of research used in this study is qualitative research, the instrument used consist of questioner, interview guidelines, observation and brainstorming. The research found 49 medical record number or medical record file has been duplicated. Factor that caused the medical record number duplication are, the lack of officers knowledge, the senior advice as reference for the officers in registration process and the disfunctional of information system. Based on the result Puskesmas Kaliwates advice to included the registration officers in the training about medical record, evaluating SOP (Standart Operational Procedure) for patient registration and procurement for supporting information system which disfunctioned or error.
