Information System Design for Web-Based Health Service Monthly Report at Kencong Health Center. Vita Permatasari, S.T, M.T (As Chief Conselor) Septiane Putri Wahyuning Tiyas Study Program of Medical Record Departement of Health ABSTRACT Activities of recording and reporting general data, facilities, personnel and health service efforts at the health center aim all data from the activities result of health center can be recorded and reported to the levels above according to the needs correctly, periodically and regularly to support the management of health community. In recording monthly reports at Kencong Health Center, reports that need to be made include LB 1 (morbidity data), LB 2 (medicine data), LB 3 (nutrition data, KIA, immunization), LB 4 (data on Health Center activities). This type of research is qualitative research with data collection and retrieval processes carried out by interviews, observation, and documentation. The method used is the waterfall method with the stages of analyzing and defining the needs, system and software design, implementation and unit testing, integration and system testing. The programming language are PHP and script editor are Notepad++. The results obtained is a web-based health service monthly report information system at Kencong Health Center.
