ABSTRACT

Hospital is health services which have suport unit called medical record. Medical record is a record file and document contain patient’s identify, examination ,treatment,actions and others services provided by health care facilities. In 2015 the average number of incomplete medical record at RSIA IBI Jember amounted to 11.23%. The incomplete medical record files at RSIA IBI Jember have some impacts, such as the discontinuity of information. Which has effect late reporting. This study aims to analyze the factors causing incomplete medical record of inpatient units at Srikandi Jember Mother and Child Hospital. This research uses qualitative methods of data acquisition. Collection data through interview, observations,usg and brainstorming. The result research obtaining the priority of the causes of incompleteeness of medical records is the lack of discipline of nursers and midwife. Alternative solution to minimize the impact is provide rewards and punistment to the officer so it cab motivate them and medical record system run better.
