ABSTRACT
The patient`s medical record file number is The patient`s medical record file number at the time of admission patien number. by interviewing with the head of medical record unit RSUD Dr.Haryoto Lumajang that problem often encountered the existence of duplicate of medical records number. There was even an inpatient medical record file with 1 patient name having more than 1 medical record number. The purpose of this research is to analyze the factors and priorities causing problem of duplication medical record number inpatient RSU. Dr.Haryoto Lumajang. The research is qualitative by using data collection method of Fishbone method, CARL, observation, interview, and brainstorming. . The results of this research are Less of officer`s at the front office, The ignorance of officer`s about the numbering system, has`nt education yet for graduates of medical record, Patients do not bring KIB, Officers do not use Standart Procedure Operational, has not been socialization Standart Procedure Operational, hospital have not used the KIUP, The System Information of Hospital error in billing system, and has not been system information of hospital update. And The purpose of this research priorities problem is patients do not bring KIB with score 625.
